Frequently asked questions

For those with statutory health insurance, the costs for the online doctor’s visit are covered by your health insurance. Privately insured persons will receive an invoice as usual, which they can submit to their insurance company.
After providing telemedical treatment, doctors can issue electronic prescriptions (including health insurance prescriptions, private prescriptions, referral prescriptions) and sick certificates for work via TeleClinic in accordance with the applicable legal requirements and regulatory framework. These are legally valid and recognised by employers and pharmacies. Please note that a sick certificate can only be issued for a maximum of 3 days.
Once the doctor has issued the electronic prescription for you, you can redeem it directly using the app. You can choose an online pharmacy to deliver to your home or visit a local pharmacy. Your prescription is, of course, available via the gematik e-prescription app or the electronic health card (eGK).
On average, you can speak to a doctor within one hour. This may vary slightly depending on how busy we are and the time of day.
First, send us your request via the online form or in the app. You will be notified when the online consultation begins and can join the conversation via the app. The connection with the doctor will be established as soon as they have also joined the online consultation. During the consultation, the doctor will take as much time as necessary to address your concerns until all important questions have been clarified and will then issue all important documents to you in the app.

You don't have to leave your bed when you're already feeling unwell, and you don't infect anyone on the way to the doctor or in the waiting room. I think that's great!

Guide to Back Pain

Abstract

  • Back pain can occur temporarily, become chronic, or even be a symptom of a serious illness.
  • Factors such as frequency, intensity, and the location of the pain must be taken into account.
  • Common causes include tight or shortened muscles, inflammation, or conditions such as osteoporosis or ankylosing spondylitis.
  • If you experience back pain, you can often take matters into your own hands and prevent the pain through certain sports and back exercises.
  • Doctors use a variety of methods to treat back pain, such as heat therapy, chiropractic care, electrotherapy, and exercise therapy.

Definition

When it comes to back pain, a distinction is made between:

  • Specific pain in the ruffles
  • Nonspecific Back Pain

Other factors that play a role include:

  • The timeline of the symptoms
  • The Severity of the Pain
  • The Frequency of Complaints
  • The areas where the pain occurs
Nonspecific Back Pain
  • There is no specific cause of back pain that requires treatment
  • At times, those affected suffer only from overstretched, tense muscles
Specific Back Pain
  • Other illnesses can trigger the symptoms or cause the back pain itself; for example, due to damage to the spine
Timeline of Symptoms
  • Acute: newly occurring symptoms that last no more than six weeks
  • Subacute: Pain lasts longer than six weeks but less than twelve weeks
  • Chronic: Back pain that lasts longer than three months
Severity of the pain
  • Pain intensity ranges from levels such as “barely noticeable,” “moderate,” and “severe” to “unbearable”
Areas where the pain occurs
  • Lower back: the area under the greatest strain; muscle tension, inflammation, herniated discs
  • Mid-back: Muscle strain, skeletal deformities
  • Upper Back/Neck: Muscle Tension and Stiffness

Causes

Back pain can be caused by a wide variety of factors. Common causes of back pain include:

  • Tense back muscles caused by stress, sedentary work, lack of exercise, or a cold draft
  • Inflammation
  • Blockages in the vertebral joints
  • Structural changes in the spine, such as degeneration or osteoporosis
  • A Herniated Disc
  • Vertebral Fractures
  • A tumor in the spinal region
  • Metastases in the spine from another primary tumor
  • Poor posture
  • Depression is often accompanied by back pain and sleep problems
  • Incorrect Training in Sports or Competitive Sports
  • Pregnancy
  • Ankylosing Spondylitis
  • Osteoporosis

Self-Help

To support your back, there are a few things you can do on your own:

  • Lose excess weight —this takes the strain off the body.
  • Regular exercise not only strengthens your muscles, it also helps reduce stress.
  • Make sure to wear comfortable shoes—it’s good for your spine.
  • Replace your mattress if necessary. As a general rule, you should buy a new mattress after eight to ten years.
  • Strengthen the transverse abdominis muscle by pulling your navel toward your spine. Continue to breathe regularly as you do this. This will provide better support for your lumbar spine.
  • When lifting heavy objects off the floor, squat down and, if possible, keep the object close to your body. It’s important to keep your pelvis stable, your back straight, and your abdomen tucked in.
  • Avoid staying in the same position for more than 30 minutes. Move around and stretch your muscles.
  • After an hour of screen time, stand up, walk around for a bit, and get your body moving.
  • Avoid putting strain on one side.

Therapy

Depending on whether you have specific or nonspecific back pain, different treatment options are available:

Treatment Options For nonspecific back pain
For acute symptoms
  • Take it easy for only a short time, since people often end up in awkward positions when lying down
  • Get active! Your muscles and spinal discs need exercise
  • Short-term use of pain relievers
  • Avoid standing or sitting for long periods, lifting, and moving forward and backward
  • Pain Management with Anti-Inflammatory Medications
  • Heat Therapy
  • Relaxation techniques, e .g., progressive muscle relaxation according to Jacobson
For subacute symptoms
  • Heat Therapy
  • Relaxation techniques, e.g., progressive muscle relaxation according to Jacobson
  • Current forms of treatment are being intensified
  • Technical or neurological tests are performed if the symptoms still persist after six weeks
For chronic symptoms
  • Further technical investigations
  • Looking for Signs of Mental Distress
  • Multimodal pain management is used for mental health conditions
  • Physiotherapy
  • Posture Correction
  • Massages
  • Antidepressants
  • Opioids
  • Acupuncture
  • Painkillers

Jacobson’s progressive muscle relaxation is a relaxation technique. Individual muscle groups from head to toe are tensed one after another for a few seconds—ideally for five to ten seconds. Afterward, you release the tension and relax for 30 to 45 seconds.

If you have any pre-existing conditions, be sure to check with your doctor to see if this form of therapy is right for you!

Type of Therapy For specific back pain
Medication
  • Anti-inflammatory drugs (NSAIDs)
  • Painkillers such as paracetamol
  • Weak opioids for short-term use
  • Antibiotics for Bacterial Infections
  • Specific medications depending on the underlying condition causing the pain, such as ankylosing spondylitis or osteoporosis
Exercise Therapy
  • Back exercises designed to stretch and build muscle in the abdomen and back
  • Swimming
  • Your doctor will recommend the appropriate form of exercise for you
Massages
  • Traditional Massages
  • Underwater Massages
  • Connective Tissue Massages
  • Tension is released, blood circulation is improved, and the body relaxes
Electrotherapy
  • Blood circulation
  • Metabolism Boost
  • Relieves pain
Chirotherapy
  • Disorders of the musculoskeletal system are treated by the doctor using their hands
  • Muscles are worked through pressure, tension, counter-tension, stretching, and relaxation
Thermotherapy
  • When applied cold: anti-inflammatory
  • With Heat: Relaxing the Muscles
Support Corset
  • Provide support and stability to the lumbar vertebrae and relief to overworked muscles
Surgical procedures, for example, for:
  • Vertebral Fractures
  • Bacterial Spinal Infections
  • Nerve compression
  • Artificial disc replacement
Psychological Therapy
  • Behavioral Therapy Methods
  • Psychoanalytic Methods
  • Relaxation Techniques
  • Meditation
  • Biofeedback
  • Hypnosis
  • Pain Management Programs

How TeleClinic can help

Our doctors can help you quickly and easily obtain a second opinion to ensure an accurate medical assessment of your acute, subacute, or chronic back pain. The important distinction between “nonspecific” and “specific” lower back pain can be easily made via telemedicine with your cooperation as a patient. If specific tests or treatments are needed that cannot be performed via telemedicine, the doctor can also explain these options to you and help arrange them.
