Doctors of gynecology: a versatile profession
Doctors who choose to specialize in gynaecology care for patients from puberty to old age. The work of gynecologists includes the diagnosis and treatment of gender-specific diseases as well as preventive counseling and reproductive medicine for pregnancy, birth and beyond. The work is carried out on an outpatient or inpatient basis and also requires surgical qualifications.

How gynecologists examine and diagnose women
A gynecologist usually examines patients at least once a year. From the age of 45, two examinations per year are recommended. This is not just about diagnosing acute illnesses, but also about advice and prevention.
The consultation between doctor and patient should clarify questions about the patient’s life situation and general health. Possible existing or new, possibly chronic illnesses should be discussed, and the gynecologist should also be aware of habits (smoking, alcohol consumption) as well as medication taken.
The examination includes measuring blood pressure and determining body weight, followed by a palpation of the breasts and female reproductive organs – often in conjunction with an ultrasound. A smear test of the uterine lining for the early detection of cancer is also part of the examination at least once a year for adult women. An examination of the rectum is also part of the routine examination from the age of 50.
If a disease is suspected, the blood or urine is analyzed. The results are usually evaluated in a laboratory. If there are palpation findings in the breast, the gynecologist refers the patient to a radiologist who performs an X-ray examination of the breast, known as a mammogram.
Cancer screening – when should you take advantage of it?
From the age of 20, women are entitled to an annual palpation examination of the genitals in combination with a smear test. From the age of 30, an annual breast palpation is included in the scope of benefits. From the age of 50, mammography every two years and an annual palpation of the large intestine, including a check for occult blood in the stool, are also part of preventive care. As a general rule, all women should make use of the preventive measures to which they are entitled, as excellent treatment results can be achieved for many cancers if they are diagnosed in good time.

Women of all ages can suffer from gender-specific diseases that can be treated or even prevented and detected at an early stage through preventive measures. Treatment methods in gynecology
The way in which a disease is treated depends on the findings. The broad field of gynecology requires the external treatment of diseases with gels or creams, for example in the case of vaginal fungi. However, drug therapy, as required for sexually transmitted diseases or infections, is also part of the specialty. Sub-areas include oncology and senology, in which appropriate therapeutic measures are combined with surgical procedures to remove affected tissue in accordance with the diagnosis.
Another field in which a gynaecologist must have expertise is the treatment of reproductive disorders, especially of the mother, the monitoring of pregnancy and the diagnosis and treatment of diseases of the mother and the unborn child. Here, the gynecologist cooperates with other doctors on a case-by-case basis.
A very special therapeutic field is reconstructive medicine, which offers help to patients and people after accidents as well as victims of (sexual) violence. Surgical reconstruction goes beyond purely cosmetic surgery and in many cases requires collaboration with specialists in trauma treatment. This is particularly necessary when the patients concerned are very young.
Most gynecologists actually meet almost all the requirements for diagnosis and treatment in this specialty. Only reconstructive surgeons and trauma specialists are qualified through appropriate further training and cannot be found in every practice or clinic.
When deciding on a gynecologist, patients should check in advance whether the gynecologist in question has the experience that is necessary in the respective case – for example, in the care of pregnant women with chronic illnesses or disabilities.
Going to the gynecologist for the first time
Parents of young girls should be aware that sensitive and at the same time fact-oriented education not only about sexuality, but also about the associated feelings of shame and insecurities is the best way to prepare their child for menstruation and growing up. A first visit to a gynecologist is also part of this, at the latest when a young girl wants to use birth control pills for the first time, for example. Choosing a gynecologist who can advise and explain in an objective and careful manner lays the foundation for later willingness to consult the doctor for regular check-ups!
Questions & answers for the gynecologist
When should I see a gynecologist?

The gynaecologist also helps with the diagnosis of pregnancy or can initiate appropriate measures if the desire to have children remains unfulfilled. Gynaecologists can also help if a sexually transmitted disease is suspected.
How quickly can I get an appointment with the gynecologist?
There are currently over 18,000 gynecologists working in Germany. They provide advice in their own practices, but in many cases also attend births in a clinic where they have beds available. How long you have to wait for a doctor’s appointment depends on the situation, as pregnant women and emergency patients are generally given priority. For appointments for regular and preventive examinations, however, you do not have to expect long waiting times; an appointment can often be made within two weeks, especially if the patient is flexible in terms of time and also considers the morning.
Do I need a referral to a gynecologist?
Referrals have no longer been mandatory since 2013. However, it is still worth asking your GP for a referral, especially if the diagnosis and treatment later requires further specialists. And one more thing: thanks to the referral, the GP receives a copy of all findings and thus keeps track of the entire medical history.
I have never been to a gynecologist and feel self-conscious. What can I do?
If you are reluctant to see a gynecologist, you should bear in mind that you will benefit from regular check-ups. You should also not ignore pain or other complaints. If young women in particular are afraid of talking to a male doctor, they can open up more easily and more light-heartedly with a female gynecologist.
Does a gynecologist also help in traumatic situations?
Gynaecologists are obliged to maintain confidentiality. If you want to confide in your doctor because you have had a traumatic experience or fear possible health consequences such as an HIV infection, the doctor can advise you carefully and help you to get clarity.
