Guide to erectile dysfunction
Abstract
- Erectile dysfunction can occur once, but it can also occur more frequently. However, a doctor should be consulted if symptoms persist or certain other factors, such as side effects of medication, psychological stress, etc., are present.
- There are a variety of different treatment options, for example with medication or (psychological) counseling sessions.
When do we start talking about erectile dysfunction?
Erectile dysfunction (ED) means that in more than two thirds of cases, a man is unable to achieve or maintain an erection that is sufficient for sexual intercourse or masturbation. If these problems persist for at least six months, they are referred to as erectile dysfunction. So if it “doesn’t work” from time to time, it is not yet a disorder requiring treatment.
Symptoms
The following symptoms may indicate erectile dysfunction:
- The absence of erections during the sleep phase or in the morning indicates physical causes. In a healthy man, however, erections occur during certain phases of sleep.
- The penis does not get hard at all or not hard enough to be able to have sexual intercourse.
- Even during masturbation, the erection stops completely or partially.

Causes
Erectile dysfunction (commonly known as “impotence”) often develops gradually and occurs both during intercourse with a partner and during masturbation.
Possible causes are above all
- Vascular changes
- Consequences of another disease that interfere with blood circulation:
- Sugar disease(diabetes mellitus)
- Vascular disease (arteriosclerosis)
- Neurological disease (polyneuropathy)
- High blood pressure
- Side effects of medication taken, such as
- Beta blockers for high blood pressure
- Draining medication
- Antidepressants
- Consequence of an injury, for example a pelvic fracture
- Sequence of operations
- Radical prostatectomy for prostate cancer
- Operations on the rectum for rectal cancer
- Herniated disc
- Surgery or radiotherapy in the pelvic area
- Smoking, increased alcohol consumption, obesity and insufficient exercise put a strain on the blood vessels and therefore on potency.
- Hormonal disorders, such as testosterone deficiency, often also with increasing age, can be the background.
- Psychological stress or pressure that is either triggered by everyday situations or life situations or is based on anxiety or burdensome expectations towards the partner.
Are erectile dysfunctions dependent on age?
The likelihood of erectile dysfunction increases significantly with age. The table provides an overview of erectile performance by age group.
- Around 20: maximum sexual performance
- Around the age of 40: Erectile strength decreases more and more
- Between 50 and 60: men often suffer from mild to moderate erectile dysfunction
- With increasing age, the proportion of men affected rises to up to 57%
What treatment options are available?
Erectile dysfunction is not a life-threatening disease. Nevertheless, it is a burden on well-being, quality of life and a partnership. Research into the causes is the primary focus in order to find an appropriate form of therapy and, above all, to rule out or treat other diseases.

PDE-5 inhibitors
Phosphodiesterase-5 inhibitors (PDE-5 inhibitors) inhibit an endogenous enzyme that breaks down a specific neurotransmitter. The messenger substance is produced in greater quantities during sexual arousal and promotes blood flow in the penis. If the enzyme is now blocked by the medication, more messenger substances remain. This increases blood flow to the penis. The penis hardens and the erection lasts longer. PDE-5 inhibitors only work during sexual arousal. They do not stimulate desire per se. Further erections are possible within the duration of action, even if the first erection ends with ejaculation. The duration of effect varies depending on the medication and dosage.

- Visual disturbances; blurred image or color vision disturbances

Prevention and home remedies
If you can’t get an erection from time to time, that’s no reason to panic. Short-term private or professional worries and problems may be responsible. This can help now:
- Talk to your partner about your situation. A clarifying, understanding conversation can go a long way and resolve some of the stressful thoughts that are often the cause.
- Don’t put yourself under pressure. You don’t have to prove anything to anyone.
- Create a pleasant atmosphere and relax.
- Ask your partner to do something that excites you and talk openly about your desires and needs.

When should a doctor be consulted?
Erectile dysfunction is not uncommon and is not a dangerous disease. However, you should make sure that there are no other illnesses present. If you notice that the problems persist over a longer period of time, consult a doctor (e.g. a urologist). If one or more of the points listed above apply to you, it is also advisable to consult a doctor. He or she will suggest an individual treatment for you. In most cases, treatment with PDE-5 inhibitors is prescribed, which are available on prescription only.
